Ok, just a WAG, but if the mag doesn't seat, then the next round is not going to get picked up. My guess is leaning to the second round is NOT seating and the hammer is dropping on an empty chamber. Needs to be more forcefull when seating the mag is all.

Re-boot might simply mean placing the butt down, barrel up and use your boot to kick the slide handle down. I had to do that with a full grown M14 once or twice.
Power Pistol is good powder. BUT Unique measured easier in the powder dispenser. I used 6.9gn of Power Pistol for about 825fps. Unique was a 5.8 or so for 800fps. Speeds determined by Labradar.
